Search Approved Provider

WINDOW OVERVIEW

The Search Approved Provider window is used to Search for a provider that has been approved for the Selected Service. The system allows a search by:

  • Provider Organization ID number
  • Provider Organization Name
  • City or
  • City and Elementary School Area or
  • City and Zip Code or
  • County

FIELD DESCRIPTIONS:

Provider Address Group Box:

City: Enter the name of the city to view a list of all selected service providers. You may narrow the search for a list of providers in a particular city by including either a zip code or an elementary school for the city.

Elementary School: Select the name of the elementary school from those in is the drop down list. If a elementary school is selected the zip code will display (none).

Zip Code: Select the zip code from those in the drop down list. If the zip code is selected the elementary field will display (none).

or

County: Select a county from those listed in the drop down list.

or

Provider Organization:

ID#: Enter the unique system assigned ID# of the provider organization.

or

Name: Enter the name of the provider organization. Because organization names may not be unique, searching by name plus one of the address combinations will return a more precise list.

ACTION BUTTONS:

Select the Search button to begin the search using the criteria specified on this window. If one or more provider organizations meet the search criteria the List Provider Detail window opens. If no provider organization meets the criteria the ‘Provider not Found' message opens.
Select the Clear button to clear any search information. The window remains open and new criteria can be entered.
Select the Cancel button to cancel the search and return to the Detail Service Type window.
Select the Help button to display window specific help information.
