Search By SSN

The Search by SSN tab on the Vital Statistics Records tab window is used to search for Vital Statistics information using the person's social security number. The window is opened from the Person Detail window and the SSN field defaults to the person's SSN as shown on N-FOCUS.

FIELD DESCRIPTIONS:

Enter Search Information

Search by List Box:

m Birth: Select this radio button to search for the person's birth record.

m Death: Select this radio button to search for the person's death record.

m Marriage: Select this radio button to search for the person's marriage record(s).

m Divorce: Select this radio button to search for the person's divorce record(s).

Person Information Box:

SSN: This field displays the person's Social Security Number.

Results List Box:

When the information does not match Vital Statistics the message reads:

No Results Found

When there is a match the List Box displays:

Certificate Number: This column displays the certificate number. double click on the certificate number to view the Details tab for the certificate.

Type: This column displays the type of certificate(s) found.

Birth Certificate Information:

Child: This column displays the last and first name of the child as listed on the birth certificate.

Mother: This column displays the last and first name of the child's mother as listed on the child' birth certificate.

Date: This column displays the child's date of birth.

Death Certificate Information:

Deceased: This column displays the last and first name of the person as listed on the death certificate.

Spouse: This column displays the last and first name of the person's spouse.

Date: This column displays the date of death.

Marriage Certificate Information:

Groom: This column displays the last and first name of the groom as listed on the marriage certificate.

Bride: This column displays the last and first name of the bride as listed on the marriage certificate.

Date: This column displays the date of the marriage.

Divorce Certificate Information:

Husband: This column displays the last and first name of the husband as listed on the divorce certificate.

Wife: This column displays the last and first name of the wife as listed on the divorce certificate.

Date: This column displays the date of the divorce.

ACTION BUTTONS:

	Select the Search button to begin the record search process.
	Select the Clear button to remove the Name criteria. SSN and date of birth cannot be cleared.
