Name: This field displays the name of the case person.
SSN: This field displays the Social Security number of the case person.

Birth Date: This field displays the date of birth for the case person.
Tax Year: This field displays the tax year in which this information was reported. Select the appropriate tax year from the drop down box.
Payer Group Box:
Date Received: This field displays the date the information was received by DHHS.
Payer Name: This field displays the name of the financial institution or employer where this source of income originated.
