Tax Year: This field displays the year in which the unearned income was received by the client.
Payer Group Box:
Account Number: This field displays the number (if any) assigned by the payer to the payee's account.
Tax Id: This field displays the SSN or Federal Identification Number for this employer.\
Document Type: This field displays the type of form (1099 or similar document) on which the payer reported the income to the IRS.
Name: This field displays the name of the person, company or organization reporting the information.
Address: This field displays the mailing address of the department, division or individual who is responsible for maintaining and reporting this person's income.
Date Recd: This is the date the IRS record was received by DHHS.
Unearned Income Group Box:
Amount: This field displays how much income was paid to the case person by the payer for the report year indicated.
Type: This field displays the type of income being reported.
