Vital Statistics Request Status

Pending: User has submitted a request, but Central Office has not yet acted on it

Verified: Verification request was verified. Any necessary data may be corrected on the Detail window. An alert will be created when this status is set.

Mailed to Vital Statistics: Central Office staff has sent the request to Nebraska Vital Statistics or another state’s Vital Statistics.

Certificate Received from Vital Statistics: Certificate has been received in Central office

Certificate Returned to Worker: Certificate has been routed to the worker who requested it.

Request Rejected: Request cannot be processed due to additional information needed. This status will also have a reason code. Most of the communication on the rejections will be completed via inter-office communication or e-mail.

Rejected Status Reasons:

ASD-46 not attached
Person not found
More information required for other State-specified information is on returned document
Photo ID for person is required for this State
Application for certificate is being returned for client’s signature
State Ward Court Order is required by the State where the certificate is located.
