Create a Transitional Living Plan
Creating an Independent Living Plan is done from the Detail Program Case window or by selecting the IL button on the Detail SDM Case Plan window. If an TLP Plan already exists, the begin date of a new plan can not be prior to the end date of the old plan.
- From the Detail Program Case window, highlight the person for whom the TLP is to be created.
- Select the TLP
button. The Detail Transitional Living Plan window appears. - Enter the begin date for the TLP. The date on the TLP can go back as far as the begin date of the Program Case. This date cannot be a future date.
- Enter the end date for the TLP. The end date cannot be more than 6 months from the begin date.
- Select the Out Select Arrow
to add plan participants. - The Completed By will default to the worker entering the TLP. If a different worker needs to be added, use the Out Select Arrow
to change worker. - Click Save
icon . Once Saved, the TLP push buttons enable. - Highlight the youth's name for whom the plan is being written and select the Plan/Goals
button to enter a goal for the youth. - Select the NYTD Provided Service
button to enter information about the service provided to the youth. - Select the Youth Need Questions
button to answer questions about the youth’s needs. - Select the Youth Need Summary
button to view a summary of the Youth Need Questions answers and narratives. - Select the Progress
button to add progress narratives for the TLP Goals. - Select the Status History
button to view the Transitional Living Plan Status History. - Select the Narrative
icon and add the required narratives. - Select the Save
or the Save and Close
icon to return to the Detail Program Case window.
Note: Once the Transitional Living Plan is complete, go to the Detail Transitional Living Plan window. Update Status from the Detail Menu.
