Mexican Consulate Notification Form

HHSS has entered into a contract agreement to notify the Mexican Consulate when a child born in Mexico is made a State Ward.  The form will pull some information in from what is on N-FOCUS, such as the child's name and birthdate,  and casemanager information.  

This form is sent to the Consulate of Mexico, 3552 Dodge Street, Omaha, NE.  68131.

1. This form is accessed through the CFS program case.  
2. On the Detail Program Case window, select the Correspondence icon .
   Result: The Search for Correspondence window will open.
3. Select the New pushbutton.
4. On the Create Correspondence window, scroll to find the Mexican Consulate Notification form.
