Create a New SDM Family Strengths and Needs Assessment (FSN)

Note: Prior to documenting the SDM FSN Assessment, it is required that the SDM Household has been documented in N-FOCUS.

 

  1. Navigate to the Detail Program Case window.
  2. Select the SDM icon.

    Result:  The SDM Household Summary/Detail window displays.

    Note:  This window should have at least one Household Name listed.

  3. Select appropriate Household Name in the group box.

    Result:  The various push buttons enable, and information in the SDM Household Group box displays, including Referral Date and other Household members.

  4. Select the FSN Asmnt push button.

    Result:  The Detail FSN Assessment window displays.

    Note:  If other FSN Assessments have been completed for this household, the List FSN Assessment window displays. You will have to select the New icon to complete a new FSN Assessment.

  5. Enter the appropriate Assessment Date.
  6. Select the Completed By Out Select arrow.

    Result:  The Search Office window displays.

    Note:  The Office field defaults to the office that corresponds to the current user’s logon ID and position. If you wish to select a different office, continue with Step 7.  If you wish to select the default office proceed to Step 12.

  7. Select the Office Out Select arrow.

    Result:  The Search Office window displays.

  8. Type in the appropriate Office Name.
  9. Select the Search push button.

    Result:  The List Office window displays.

  10. Select the office you wish to view.
  11. Select the blue return arrow.

    Result:  The Search Office Position window displays.

  12. Select the Search push button.

    Result:  The List Office Position window displays.

  13. Select the position that you wish to assign to the FSN Assessment.
  14. Choose the blue select arrow.

    Result:  The Detail FSN Assessment window displays.

  15. Select the Save icon.

    Result:  The Caregiver Rating, Caregiver Rating Summary, Child Rating and Child Rating Summary push buttons are enabled.

  16. Select the Caregiver Rating push button.

    Result:  The Caregiver Rating Details window displays.

  17. Select the appropriate rating for each Caregiver identified.
  18. Select the Save & Next push button.

    Result:  The next Rating Item displays.

  19. Repeat Save & Next until all 9 Rating Items have been documented.

    Note:  The # 9 Item is optional, but if selected, you must also enter text in the Comment field.

    Note:  You can view the Ratings entered by selecting the Rating Summary push button.

  20. Select the Save & Close icon.

    Result:  The FSN Assessment window displays.

  21. Select the Child Rating push button.

    Result:  The Child Rating Details window displays.

  22. Select the child(ren) for whom the Rating applies..
  23. Select the appropriate Child Rating
  24. Select the Save & Next push button.

    Result:  The next Rating Item displays.

  25. Repeat Save & Next until all 12 Rating Items have been documented.

    Note:  The # 4 Item requires the Additional Education Items also be documented.

    Note:  The # 12 Item is optional, but if selected, you must also enter text in the Comment field.

    Note:  You can view the Ratings entered by selecting the Rating Summary push button.

  26. Select the Save & Close  push button.

    Result:  The FSN Assessment window displays.

  27. Select the Prioritization push button.

    Result:  The Caregiver Prioritization and Strength window displays with the Caregiver Rating Items previously identified.

  28. Select the Needs Questions push button.

    Result:  The Caregiver Needs Questions dialogue box displays.

  29. Enter the information for all the questions regarding the first Need Item.
  30. Select the Next Need  Item push button.

    Result:  The next Need Item displays.

  31. Repeat the Next Need Item until all of the Need Items have been documented.

    Note:  The Next Need Item push button disables when completed.

  32. Select the OK push button.

    Result:  The Caregiver Prioritization and Strength window displays.

  33. Select the Strength Questions push button.

    Result:  The Caregiver Strength Questions dialogue box displays.

  34. Enter the information for all the questions regarding the first Strength Item.
  35. Select the Next Strength Item push button.

    Result:  The next Strength Item displays.

  36. Repeat Next Strength Item until all of the Strength Items have been documented.

    Note:  The Next Strength Item push button disables when completed.

  37. Select the OK push button.

    Result:  The Caregiver Prioritization and Strength window displays.

  38. Select the OK push button.

    Result:  The Detail FSN Assessment window displays.

  39. Select the Critical Needs  push button.

    Note:  The Critical Needs push button will only be enabled if in the Prioritization area, a Need has been identified as being related to a Safety Threat, but it has not been prioritized for the Case Plan.

    Result:  the SDM Narrative window displays.

  40. Select the first Narrative Type and enter the Narrative text.
  41. Select the Save & Next icon to enter narratives for all of the Narrative Types listed.
  42. Select the Save & Close icon.

    Result:  The Detail FSN Assessment window displays.

  43. Select the Save icon.
  44. Select Actions – Update Status.

    Result:  The Confirm Save message displays, asking if you want to change the  Assessment status to, “Ready for Review”.

    Note:  If required information has not been documented, the Status Change Edit Errors Dialogue box displays. All of the edit errors must be corrected in order to set the status.

  45. Select Yes.

    Result:  The Detail SDM FSN Assessment window displays with the Status of Ready for Review.

    Note:  Supervisor will be responsible for setting Status to ‘Final’

  46. Close back to the Detail Program Case window.
