Complete a YLS Assessment

On the Person Detail window, click on the YLS icon.   If no assessment has been done for the child, the window will open to the YLS Assessment Detail window.

If there is an existing assessment for the child, the window will open to the YLS Assessment List window.  Assessment statuses are:

• Draft:  This is the only assessment where changes can be made.
• Pending Approval:  
• Closed:  These are sorted by date.  

Assessment Date:  This defaults to the current date, but may be changed to an earlier date.

Documented By:  This defaults to the person initially entering the information.  It may be changed by selecting the blank out arrow.   The name is to be the worker who actually conducted the assessment.

Assessment Type: There are 5 assessment types that describe the juvenile's situation at the time the assessment was conducted:

• OJS Evaluation Only:  Juvenile is a ward of the state for Evaluation purposes only.
• Direct Commit:  Juvenile is ward of the state but remains at home or is placed in an alternate placement (not a YRTC).
• YRTC: Juvenile is a ward of the state but has been placed in a Youth Treatment Facility.
• Parolee: Juvenile Ward was placed in a YRTC facility but has returned home.
• Completed by  External Agency

There are eight domains in each assessment.  Each domain has it's own characteristic detail window for scoring purposes.  Double click on the domain name to open the window for that domain. The domains are:

• Offenses/Disposition
• Family/Parenting
• Education/Employment
• Peer Relations
• Substance Abuse
• Leisure/Recreation
• Personality/Behavior
• Attitudes/Orientation
