Cultural Plan Tribe Details

This window is accessed by selecting the button on the Detail Cultural Plan window. The Cultural Plan Tribe Details window is used to document the youth's tribal information, the Cultural Plan Motion and Court Decision in addition to information regarding the Tribal Membership Inquiry Form.

 

FIELD DESCRIPTIONS:

Child's Name:  The name of the youth for whom the Cultural Plan is being written.

Date of Birth: The youth's birth date.

Tribal Group Box:

Tribe Name: The name of the tribe the youth is associated with.

Enrollment Number: The tribal enrollment number.

Motion/Hearing/Court Decision Group Box:

Motion Date: The date the motion was filed.

Hearing Date: The date the hearing was held.

Court Decision: The decision of the court.

Reason, if motion denied: Enter narrative explaining the reason for a denial of the Cultural Plan.

 

ACTION BUTTONS:

	Click the Max icon to open the Maximize Narrative window.
	Click this icon to spell check the narrative.

 

Tribal Membership Inquiry Form Group Box:

This group box contains information received from the Tribal Membership Inquiry Form.

Yes, sent: Select this radio button if the Tribal Membership Inquiry Form as been sent.

No, not sent/not current: Select this radio button if the Tribal Membership Inquiry Form has not been sent or is not current.

Last Sent Date: The date the most recent Tribal Membership Inquiry Form was sent.

Reason, if not sent/not current: Enter, in narrative form, the reason the Tribal Membership Inquiry Form was not sent or is not current.
