Service Authorization List

WINDOW OVERVIEW:

The Service Authorization List window displays all established Service Selections for one or more persons in a specific Program Case. The listed services indicate the Case Person is eligible to receive that particular service. If a Service Authorization has also been created, the Service Authorization dates and provider name will display.

How Do I:

Service Authorization

FIELD DESCRIPTIONS:

First:This field displays the first name of the person who is eligible/authorized to receive the service.

Service: This field displays the name of the service the person is eligible to receive.

Auth Begin:This field displays the begin date of the Service Authorization.

Auth End:This field displays the date the Service Authorization is to end.

Authorized Provider: This field displays the name of the Person or Organization that has been authorized to provide this service. If a Service Authorization is for a reimbursement to a Case Person, this field will not have a name.

MI:This field displays the middle initial of the eligible person

Last: This field displays the last name of the eligible person

Ext:This field displays the name extension for the eligible person.

Date Created:This field displays the date the Service Authorization was created. This date may be different than the Service Authorization begin date.

Last Notice Created:This field displays the date the last notice was created for this Service Authorization.
