Death Request Detail

WINDOW OVERVIEW:

The Death Request Detail window displays the information entered in the search for the death certificate.  If necessary, the request can be updated on this window.  

The window opens when a person line is double clicked on the Request List window.

FIELD DESCRIPTIONS:

Request Type:  This box displays the type of certificate requested.

Person Number:  This box displays the agency related person number assigned to the person.

Status:  This box displays the current status of the request.  This field can be updated by selecting another status from the drop down list.

Status Date:  This box displays the effective date for the status.   To update enter the date of select the calendar icon to open a calendar of the current month.

Status Reason:  This box displays when the update button is selected.  Click on there reason for the update from the list in the drop down box.

Person Box:

First Name:  This field displays the person's first name as listed on the previous request.  This is a required field.

Middle Name:  This field displays person's middle name if they have one.

Last Name:  This field displays the person's last name.  This is a required field.

Ext:  This field displays the person's name extension if they have one.

Birth Date:  This field displays the person's date of birth.  This is a required field. The calendar icon may be used to find the date.  

Death Date:  This field displays the person's date of death (MM-DD-YYYY).  This is a required field. The calendar icon may be used to find the date.  

SSN:  This field displays the person social security number.

City:  This field displays the name of the city where the person died.  This is a required field.

State:  This field displays the name of the state where the person died.

County:  This field displays the county where the person died.


Certificate Number:  This field displays the certificate number after it is received.

Request List:

This list displays all of the requests for this person

Status Date:  This column displays the each date a change was made to the request.

Status:  This column displays the status of the request.

Status Reason:  This column displays the reason for the status.

Create User Id:  This column displays the name of the staff person that created or updated the request.

Warrant Boxes:

These boxes display when there is an out of state request.

Warrant Number:  This field displays the number on the check sent to the other state.

Warrant Date:  This field displays the date the check was created.

Warrant Amount:  This field displays the amount of the check.

ACTION BUTTONS:

 Select the update button to add changes to the window information.
 The Print button is available for out of State requests.  Select it to open the Print window so the page can be printed.
