FIELD DESCRIPTIONS:

Identifying information:

SSN:  This box displays the N-FOCUS Social Security Number for the person.

Birth Date:  This box displays the person's N-FOCUS date of birth.

Name:  This box displays the person's N-FOCUS name.

Gender:  This box displays the person's N-FOCUS gender code.

Eligibility Information:

Continuous Period:  This box displays the begin date of the most recent continuous period of entitlement to Part B.

Enrollment Period:  This box displays the period that the person enrolled in Part B.

Premium Amount:  This box displays the amount of the Part B premium.

Premium Payer:  This box displays the

Private payer Group Payer Enrollment.premium payer.  If the premium payer is the beneficiary this box is blank.

Third Party Information:

Third Party Category:  This box displays the
Civil,

Statetype of third party payer.

Third Party Start Date Information:

This area displays up to three occurrence of third party payment.

Start Date:  This box displays the date for which a third party accepted liability of Part B premiums.

Basis:  This box displays the basis of Part B eligibility coverage.

Third Party Start Stop Information:

This area displays up to three occurrence of third party payment termination.

Stop Date:  This box displays the last month for which a third party no longer accepted liability for Part B premiums. The third party has paid Part B premiums due for the month indicated.

Non-Covered Reason:  This box displays the reason the Part B coverage ended.
