FIELD DESCRIPTIONS:

Identifying information:

SSN:  This box displays the N-FOCUS Social Security Number for the person.

Birth Date:  This box displays the person's N-FOCUS date of birth.

Name:  This box displays the person's N-FOCUS name.

Gender:  This box displays the person's N-FOCUS gender code.

Claim Information:

Claim Number:  This box displays the Claim Number used for Medicare benefits..

Date Received:  This box displays the date the record was received in the N- FOCUS system.

Continuous Period:  This box displays the begin date of the most recent period of continuous entitlement to Part A.

Enrollment Period:  This box displays the type of enrollment during which the person enrolled in Part A.

Coverage Type:  This box indicates whether the person has free Part A coverage or must pay a premium.

Premium Amount:  This box displays the amount of the Part A Medicare coverage when a premium is required..

Third Party Category:  This box displays the

Conditional type of third party payer.

Third Party Premium Payer:  This box displays the
