Create Request - Marriage

WINDOW OVERVIEW:

The Create Request - Marriage tab is used to request a Marriage certificate or verification of birth on the Official Nebraska Government Website for Vital Statistics. You will be able to Search by Name or Search by SSN. To open this website, click the Interface icon on the Person Detail window.

The Interfaces Menu will display.

Select the VS (Vital Statistics) icon.

The Official Government Website for Vital Statistics Records will display.

The following tabs are available from the Verifications and Certificate Requests tab :

• Create Birth Request

• Create Death Request

• Create Marriage Request

• Create Divorce Request

FIELD DESCRIPTIONS:

Enter Marriage Certificate Information

Select Request Type:  Click on the appropriate radio button.

m NE Verification

m NE Certified

m Out of State

Groom Box:

First Name:  Enter the groom's first name.  This is a required field.

Middle Name:  Enter the groom's middle name if he has one.

Last Name:  Enter the groom's last name.  This is a required field.

Ext:  Enter the groom's name extension
Jr. Sr. III. etc. if he has one.

Bride Box:

First Name:  Enter the bride's first name.  This is a required field.

Middle Name:  Enter the bride's middle name if she has one.

Last Name:  Enter the bride's last name.  This is a required field.

Ext:  Enter the bride's name extension
Jr. Sr. III. etc. if she has one.

Maiden Name: Enter the bride's maiden name.

Marriage Boxes:

Marriage Date:  Enter the date of the marriage.  This is a required field. The calendar icon may be used to find the date.  

City:  Enter the name of the city where the couple married.  This is a required field.

State:  Enter the name of the state where the couple married.

County:  Enter the county where the couple married.
