Create Request - Divorce
WINDOW OVERVIEW:
The Create Request - Divorce tab is used to request a divorce certificate
or verification of birth on the Official Nebraska Government Website for Vital Statistics. You will be able to Search by Name or Search by SSN. To open this website, click the Interface icon
on the Person Detail window.
The Interfaces Menu will display.
Select the VS (Vital Statistics) icon.
The Official Government Website for Vital Statistics Records will display.
The following tabs are available from the Verifications and Certificate Requests tab :

FIELD DESCRIPTIONS:
Enter Divorce Certificate Information
Select Request Type: Click on the appropriate radio button.

m Out of State
Husband Box:
First Name: This field displays the husband's first name. This is a required field.
Middle Name: This field displays the husband's middle name if he has one.
Last Name: This field displays the husband's last name. This is a required field.
Ext: This field displays the husband's name extension if he has one.
Wife Box:
First Name: This field displays the wife's first name. This is a required field.
Middle Name: This field displays the wife's middle name if she has one.
Last Name: This field displays the wife's last name. This is a required field.
Ext: This field displays the wife's name extension if she has one.
Maiden Name: This field displays the wife's maiden name.
Divorce Boxes:
Divorce
Date: This
field displays the date of the divorce. This
is a required field. The calendar icon may be used to find
the date.
City: This field displays the name of the city where the couple divorced. This is a required field.
State: This field displays the name of the state where the couple divorced.
County: This field displays the county where the couple divorced.
