Create a New CHARTS Referral

Key Points:

The "DHHS is CP" checkbox is to be used only when there is a State Ward. This will list the State of Nebraska as the custodial party for the child.

The "Unknown NCP" checkbox is only to be used when the client or custodial party does not know the first and last name of the parent. Do not add any derivative of "Unknown" or "Anonymous" to the system.

The "Dependent is in an Institution" checkbox is only to be used when a child who is in a Medicaid or AABD program case has been placed in an institution or group home AND the income/resources/needs of the parent(s) are not being considered. This dependent is considered as living alone and has no CP.

Definitions:

  1. Select CHARTS Referral from the Actions drop down list on the Detail Master Case window.
  2. From the CHARTS Referral List window, select the green "New" icon. This will open the CHARTS Referral Detail window.
  3. All persons known to this Master Case (including previously selected NCP's) will appear in the List Box titled "Persons Known to Master Case". From this list, highlight the Custodial Party. Then select the Custodial Party pushup. This will move the person to the CHARTS Referral List box. Next highlight the Dependent(s). If there are more than one dependent which have the same CP and NCP, all may be highlighted. Then select the Dependent button to move to the CHARTS Referral List box. If the Non-Custodial Party was ever in this Master Case or selected as an NCP in this Master Case, the name will appear in "Persons Known to Master Case" list also and can be selected by highlighting and using the Non-Custodial Party button to move the person to the Referral List box. If the Non-Custodial Party is not listed in the "Persons Known to Master Case", use the “Add Non-Custodial Party” select arrow and add the name on the Add Person window. The NCP may be known to N-FOCUS and can be selected in Person Clearance List by using the “Use Existing N-Focus Person” button. An address window will appear after adding the NCP. If the NCP's address is unknown, select cancel. The NCP will be added to the CHARTS Referral List when you are returned to the CHARTS Referral Detail window.
  4. After all persons have been selected, check the CHARTS Referral List box to determine if everyone has the correct role before sending the CHARTS Referral. If anyone has an incorrect role, highlight and select the Remove button to remove them from the referral. Then repeat step 3 above to add the correct person to the correct role. Once a Referral has been sent to CHARTS, it will establish the roles that you identified in the CHARTS Referral List box and these cannot be changed.
  5. Select the Family Relationship icon and establish the relationship for all NCP's to both the CP and the Dependent(s). The Family Relationship List window will display all relationships. Any relationship between the CP and Dependents will need to be established in the expert system unless this is an SG or SG/MED program case. If the NCP was in this Master Case and has a Household Status of IN HOUSEHOLD, Family Relationships will need to be established in the expert system. An alleged parent where no paternity has been established is considered a "non-relative". If paternity has been established, then they are a "parent(bio)"
  6. The Good Cause will always default to "no". If Good Cause has been claimed, select the checkbox so it will notify Child Support that a Good Cause claim has been requested and a Family Violence indicator can be placed on CHARTS so address information can be protected.
  7. Indicate whether or not any Supplemental Information is being sent to CSE. This will notify them to be watching for this supplement documentation or that nothing further is being provided so they can request further information if necessary. It is important to send any supplemental information with a CSE-97 form. Either the "yes" or "no" must be selected in order to send the CHARTS Referral.
  8. Enter any Comments. There is a maximum of 250 characters for the text in the Comments.

Suggested uses for this are: other alleged parents (name, date of birth and address),partial addresses (such as - last known to be living in Hickman or his address is unknown, but he does keep in contact with his parents who reside at 404 North 4th, Yankton, SD), approximate age if the date of birth is not known (such as -approximately 39 years old), parent is deceased (such as - parent died in a car accident on 7/20/04), nicknames and other pertinent information about the NCP. You can also copy all or a portion of your case narrative and include in the Comments. If more than 250 characters are selected from the narrative, only the first 250 characters will paste into the Comments section. Please use discretion when entering comments as the 'Comments' will become part of the CHARTS permanent case history and may be used for legal proceedings.

  1. Select Send Referral from the Actions drop down list to send the referral at this time. If the program case is still pending and you do not want to send the referral, then select the Save/Close to be returned to the CHARTS Referral List window. Save/Next can also be selected which will save the existing CHARTS Referral and present another blank CHARTS Referral Detail window to enter another referral. The Referral Date will not be populated until you actually send the CHARTS Referral.
